This extension exists to allow for definition and use of value sets that are currently unbounded, unbounded by definition, including those for which it may always be impractical to set bounds. This can be done extension is not used to allow additional learning in this space, such as exploring poorly understsood consequences communicate value sets that are limited for practical reasons. The offset and total elements of alternatives. the expansion support that use case.
